• Pharmacy Benefit: 

    The Columbia Public School District has contracted with MedTrak Services to manage the pharmacy portion of the medical plan. Members enrolled in the District’s medical plan can utilize the pharmacy program with the medical plan insurance I.D. card.

     

    This program covers prescriptions filled at participating retail pharmacies. The pharmacy program also covers prescriptions filled at the Mail Order Pharmacy. Effective January 1, 2003, the new mail order pharmacy is Walgreens Health care Plus located in Phoenix, Arizona. 

     

    MedTrak website: You can access a history of your prescription drug purchases and their costs. For access to prescription drug information, go to www.medtrakrx.com and click on the “Members” tab at the top. Next, click on “Need to Register?”. For the card holder ID field, enter the member number from your medical ID card (CP00 + 5 digit CPS ID number). Enter the required information along with the user name and password you prefer and click “register.” Then, log in by using the user name and password you have chosen. Once logged in, click on “my medicine cabinet” to see a list of your prescriptions.

    Note: If members have a brand prescription filled when a generic equivalent is available, the member will pay the brand copay plus the difference in cost between the brand and the generic drug. The difference paid will not count towards the Rx copay out-of-pocket limit. 
     
    2018 Plus Plan - Pharmacy Benefit  - 100% of eligible Rx costs count towards the medical initial out-of-pocket maximum. Then, Rx copays begin until Rx copay out-of-pocket maximum is reached. See the Benefits Overview for more details.

    Plus Plan – Pharmacy Benefit – Under the Plus Plan, your prescription costs apply to the deductible. You pay 100% of prescription costs until your deductible is met. Once the deductible has been met, the cost of your prescription will become a copay until the prescription copay out of pocket limit has been met. 
     

    When you use your medical ID card and get your prescription filled, the cost information is submitted to the medical plan, and the amounts are applied to your deductible.

    You can use the mail order option to purchase three months of prescriptions, but you will be charged for the three month cost of the prescriptions. Mail order does provide additional discounts on prescription drugs you purchase.
