Update - 2018 Tax Documents:
1095-C tax documents were mailed to all recipients on February 5, 2019. Active employees may access this document via Employee Online.
The IRS deadline to provide the 1095-C tax document regarding healthcare coverage for 2018 is March 4, 2019. Both electronic and paper versions will be provided. You do not need this form in order to file your taxes, you simply need to check the appropriate box on your tax filing form regarding your healthcare coverage for 2018.
Quick note regarding Vaccines:
Members may go to any network pharmacy that is administering vaccines. Members will pay a $0.00 copay for ACA approved vaccines and the plan will pay the administration fee (no more than $25). There is a listing (found here) of preferred pharmacies. Or, check here to do a search of other local pharmacies to see if your pharmacy is in the network. If you have questions regarding this process, members should contact MedTrak at 1-800-771-4648.
