This form is to be completed by a healthcare professional (MD/DO/ARNP/PA/DC) and is required for students participating in All Sports, Spirit, or Marching Band. After the completion of a preparticipation physical evaluation, this Medical Eligibility Form must be issued to the student/parent(s)/guardian and returned to the school administration. The healthcare professional must retain a copy of the form in the patient's files for their records.

Academic success is monitored weekly, and regular attendance at all obligations is mandatory. Respectful behavior towards all individuals associated with the school is expected, along with teamwork and responsibility for uniform maintenance.
