Flu Shot Information
Each fall, Columbia Public Schools works with the Columbia/Boone County Health Department to administer free influenza (flu) vaccine to Columbia Public School students in preschool through 12th grade. The purpose of a school-based clinic is to offer families a convenient method to vaccinate children so they can remain healthy and ready to learn. This opportunity to have your child vaccinated is strongly encouraged but is not mandatory. Parents/guardians are welcome to accompany their child, but it is not a requirement.
Only the injectable (flu shot) form will be administered this year. The consent form is available in English and Spanish in the links below.
Columbia Public Schools welcomes the opportunity to partner with the Columbia/Boone County Department of Public Health and Human Services in serving the health needs of our students. The vaccine program is made possible through the generosity of MU Health Care.
It is recommended that everyone receives the flu vaccine each year. If your student is unable to receive the flu vaccination at their school's vaccination clinic, they can still receive a free flu vaccine at the Columbia/Boone County Department of Public Health. All Boone County children (6 months - 18 years) can receive a free flu vaccine at the
Health Department clinic
1005 West Worley.
Clinic hours are Monday - Friday from 8:00 a.m. - 4:30 p.m. by appointment.
If you have further questions, please get in touch with your school nurse.
