School-Based Health Services
The SCHOLAR Clinic provides primary care and behavioral health services to students in Columbia Public Schools. School-based health services bring needed care to where students spend most of their day. This partnership helps students thrive in and out of the classroom.

Registering for Services
Enrollment paperwork can be obtained at your child’s school, on Clarity Healthcare’s website, or completed electronically by email. Completing paperwork does not mean your child will be treated without your consent. You will be contacted before any services are provided to your child.
Cost
Clarity Healthcare accepts many insurance plans and provides a sliding fee scale for those without insurance. If a student qualifies for free or reduced lunch, or attends a free/reduced lunch school in the district, there is no out-of-pocket expense for services.
Frequently Asked Questions
- A student will not be seen without parent or guardian consent.
- If your child becomes sick at school, the school nurse will contact you to ask if you want your child seen at the clinic.
- You may stay at work, come to the appointment, or join by phone or telehealth.
- The provider can send prescriptions to your pharmacy.
- The provider will call you with updates after the appointment.
School-Based Billing
- If your child has insurance, insurance will be billed for services.
- A sliding scale is available for those with or without insurance.
- If your child receives free or reduced lunch, or attends a free/reduced lunch school, services are free.
- If your child has insurance and qualifies for free or reduced lunch, insurance will be billed, but there will be no out-of-pocket expense.
- An explanation of benefits from insurance is not a bill.
- If you receive a bill from Clarity/SCHOLAR Clinic and have questions, call the billing department at 573-603-1460 ext. 2380.
- If you do not have insurance, Medicaid Navigators can help you enroll in Medicaid.
