Covid Vaccine Clinic
STUDENT SCHOOL BASED COVID VACCINE CLINIC
Where: Oakland Middle School
WHEN: Monday December 6, 2021
8:00AM-10AM
VACCINE CONSENT FORMS MUST BE
- FULLY completed and SIGNED by the parent or legal guardian
- Completed for EACH CHILD (you must return a separate form for each child you want vaccinated)
- Returned to your school nurse by December 2, 2021.
Forms will be coming home with your student the week of 11/15 please ask them for these or you can click here to print one.
Extra consent Forms will be available at OMS main office but must be completed by parent or guardian
If you return the consent form, and your child receives the COVID-19 vaccine elsewhere, it is important that you notify your school nurse immediately. This will prevent your child from receiving a second dose of vaccine prematurely.
The Pfizer-BioNTech COVID-19 vaccine will be offered. The Pfizer-BioNTech Emergency Use Authorization Fact Sheet can be found at: https://www.fda.gov/emergency-preparedness-and-response/coronavirus-disease-2019-covid-19/comirnaty-and-pfizer-biontech-covid-19-vaccine
Vaccines will be given by:
Columbia/Boone County Department of Public Health and Human Services.
